Zusammenfassung

Eine palliative Betreuung von Parkinsonpatienten gewinnt zunehmend an Bedeutung. Die Symptomlast der Patienten in fortgeschrittenen Stadien ist komplex und hoch. Sie ist oft vergleichbar oder sogar größer als die vieler onkologischer Palliativpatienten. Viele Patienten in den fortgeschrittenen Phasen des M. Parkinson haben palliative Bedürfnisse. Auf Grund von langen, nur sehr eingeschränkt vorhersagbaren Krankheitstrajektorien ist eine Einbindung in gängige Palliativstrukturen nicht einfach. Insbesondere ein Abschätzen des richtigen Zeitpunktes für den Beginn einer palliativen Betreuung ist schwierig. Das Auftreten von Schluckstörungen definiert den Beginn der späten und palliativen Krankheitsphase und kann als Orientierungshilfe genommen werden. In der aktuellen Arbeit soll Bezug genommen werden auf palliativmedizinische Versorgung von Patienten mit Parkinson im speziellen die finale Phase betreffend und mit besonderem Augenmerk auf die pflegenden Angehörigen.

Abstract

Palliative Care in Parkinson’s Disease (PD) is of growing interest. The burden of symptoms of late-stage PD patients is similar or even higher than those of patients suffering from oncological diseases. However, patients suffering from advanced phases of PD have tremendous unmet palliative care needs. The disease trajectories are often difficult to define, and initiation of palliative care approach has to be chosen carefully. Swallowing difficulties define the onset of the late and palliative phase and have to be seen as a landmark for palliative care involvement. Here we provide an approach to palliative care in PD in the final phase and also care for the relatives.
