Zusammenfassung

Die Beratung und Betreuung extrem adipöser Patienten vor Indikationsstellung zu einem adipositaschirurgischen Eingriff ist eine große Herausforderung für alle Beteiligten. Die nachhaltige Umsetzung einer konservativen Adipositastherapie stellt dabei nur eine der Voraussetzungen für die Indikationsstellung für bariatrische Operationen dar. Wichtig ist die Einbindung der Patienten in ein interdisziplinäres Behandlungs- und Betreuungsteam aus Ernährungsmedizinern, Internisten, Chirurgen, Psychologen und Selbsthilfegruppen. Aufgrund der hohen Morbidität und Exzess-Mortalität extrem adipöser Patienten sind wirksame, strukturierte Therapiekonzepte einschließlich zeitgerechter bariatrischer Interventionen dringend erforderlich.

Summary

Counseling and attending of patients suffering from extreme obesity is a great challenge for all the people involved. This is especially true before making a therapeutically decision in the case of an obesity surgery The lasting implementation of a conservative obesity treatment is only one prerequisite for making a diagnosis for bariatric surgery. The integration of the patient into an interdisciplinary treatment and attendance team of ­nutritionist, internist, surgeon, psychologist and self-help group is very important. Both high morbidity and excess mortality of extremely obese patients require effective and ­sustaining treatment strategies, including bariatric surgery.
