Zusammenfassung

Bei 64 lungentransplantierten Patienten (33 Männer, 31 Frauen, mittleres Alter 45 [21-68] Jahre; postoperatives Überleben > 2 Wochen) wurde die Inzidenz, Ätiologie, Diagnostik und Therapie von pulmonalen Rundherden untersucht. Bei acht Patienten (12,5 %) entwickelten sich 1-10 Monate nach der Lungentransplantation (im Median 5,8 Monate) ein (n = 2) oder mehrere Rundherde (n = 6). In sechs Fällen handelte es sich um einen röntgenologischen Zufallsbefund, die Klärung erfolgte mittels Computertomographie und Feinnadelpunktion. Die Ätiologie der multiplen Herde umfaßte B-Zell-»Lymphome« (»posttransplant lymphoproliferative disorders«; PTLD) (n = 3), Aspergillome (n = 2) und bakterielle Abszesse (n = 1). Zwei Patienten starben an einer Sepsis (Aspergillus; Pseudomonas aeruginosa/Staphylococcus aureus), in vier Fällen kam es zu einer kompletten Remission. Die beiden solitären Rundherde verschwanden ohne spezifische Therapie nach 2-3 Wochen. - Bei Auftreten von Rundherden nach Lungentransplantation ist in jedem Fall eine rasche histologische und mikrobiologische Diagnostik und aggressive Therapie anzustreben, um die ansonsten hohe Letalität zu senken. Differentialdiagnostisch stehen PTLD und Infektionen (bakteriell/mykotisch) im Vordergrund.

Abstract

Incidence, aetiology, diagnosis and treatment of round lesions in the lungs were analysed in 64 patients after lung transplantation (33 men, 31 women; mean age 45 [21-68] years; postoperative survival > 2 weeks). These lesions were found in 8 patients 1-10 months (median of 5.8 months) after the transplantation, singly in two, multiple in six. In six patients it was an incidental finding, further elucidated by computed tomography or fine-needle biopsy. The aetiology varied from B-cell »lymphoma« (posttransplant lymphoproliferative disorder - PTLD) in three patients, aspergilloma in two, and bacterial abscess in one. Two patients died of septicaemia (Aspergillus; Pseudomonas aeruginosa/Staphylococcus aureus), while four had a full remission. The solitary lesions disappeared without specific treatment in 2-3 weeks. - If round lesions are noted after lung transplantation, rapid histological and microbiological diagnosis and aggressive treatment are necessary to combat an otherwise high death-rate. PTLD and infection (bacterial or mycotic) are the most frequent causes.
