Zusammenfassung
Bei einem 29jährigen Patienten entwikkelte sich nach unspezifischen Allgemeinsymptomen
(Adynamie, Nachtschweißneigung) über einige Tage plötzlich ein purpuraartiges Exanthem
an beiden Händen und Füßen, verbunden mit geringem Juckreiz und Ödem. Im peripheren
Blutbild fielen eine Leukopenie (2100/µl) mit Neutropenie (1100/µl), eine Thrombopenie
(81 000/µl) und eine Verminderung des Retikulozytenanteils (1 ‰) bei normalem Hämoglobingehalt
auf. Im Knochenmark zeigte sich eine fast vollständige Reduktion der Erythropoese
mit Nachweis von Riesen-Proerythroblasten. Granulo- und Megakaryopoese waren unauffällig.
Serologisch konnte bei positivem Nachweis von IgM- und IgG-Antikörpern gegen Parvovirus
B19 eine akute Parvovirus-B19-Infektion diagnostiziert werden. Das Exanthem, die Blutbild-
und Knochenmarksveränderungen bildeten sich innerhalb von einer Woche spontan zurück.
Die manopedale Purpura (petechiales »glove and sock«-Syndrom) ist möglicherweise eine
spezielle Manifestationsform einer Parvovirus-B19-Infektion.
Abstract
After a period of general nonspecific symptoms (weakness; nocturnal sweating) for
a few days a 29-year-old man suddenly developed a purpura-like rash on both hands
and feet (»glove and sock«) with mild itching and oedema. A blood count demonstrated
leukopenia (2100/µl) with neutropenia (1100/µl), thrombocytopenia (81,000/µl) and
reticulocytopenia (1 ‰), while haemoglobin content was normal. The bone-marrow showed
almost complete reduction of erythropoiesis with the presence of giant proerythroblasts.
Granulopoiesis and megakaryopoiesis were unremarkable. Positive tests for IgM and
IgG antibodies against parvovirus B19 established the diagnosis of infection with
this organism. The rash, blood picture and bone-marrow changes all regressed spontaneously,
without any treatment, within a week. The petechial or purpuric »glove and sock« syndrome
may be a special form of parvovirus B19 infection.
