Zusammenfassung
Am Beispiel eines drogenabhängigen Patienten mit Candida-Endophthalmitis wird die
neue Applikationsform des Antimykotikums Amphotericin B in liposomaler Verkapselung
vorgestellt. Wir konnten gegenüber dem bisherigen Amphotericin B die geringeren Nebenwirkungen
und die fehlende Nephrotoxizität bestätigen. Eine Erregerreduktion oder -eliminierung
wurde bei unserem Patienten vermutlich erreicht, allerdings entwickelte sich eine
ausgedehnte Traktionsamotio. Bei mykotischer Endophthalmitis halten wir in Zukunft
einen Behandlungsversuch mit liposomalem Amphotericin B in einer höheren Dosierung
für empfehlenswert.
Summary
We report this exemplary case of a drug addict with Candida endophthalmitis where
we used the new form of amphotericin B, encapsulated in liposomes. We were able to
reconfirm the reduced number of side effects and the minimized nephrotoxicity reported
by authors of other specialities. In our patient, a reduction or elimination of the
yeast was probably achieved, nevertheless, he developed a traction retinal detachment.
In future cases of fungal endophthalmitis, we recommend liposomal amphotericin B in
higher doses.
