Lamelläre Horse-Shoe Sklerokeratoplastik und Thermoplastik bei Keratokonus mit peripherer Keratektasie*

Zusammenfassung

Ziel Die am schwierigsten zu behandelnde Verlaufsform eines Keratokonus ist die periphere Ektasie der Hornhaut, die im fortgeschrittenen Stadium weder mit Brillengläsern noch mit Kontaktlinsen zu korrigieren ist. Perforierende Hornhauttransplantate müßten unmittelbar am Limbus plaziert werden. Durch ihre Limbusnähe sind solche Transplantate ganz erheblich von Immunreaktionen bedroht, und die Prognose ist deshalb primär schlecht. Um eingreifende, das Immunsystem modulierende Maßnahmen wie die Prophylaxe mit systemisch gegebenem Ciclosporin A zu vermeiden, haben wir bei einem solchen Patienten die perforierende Keratoplastik durch eine spezielle lamelläre Operationstechnik ersetzt.

Patient Bei dem 35jährigen Patienten mit beidseitiger peripherer Keratektasie betrug der beste Brillenvisus nur noch 0,1. Harte Kontaktlinsen wurden nicht mehr toleriert.

Ergebnis Nach lamellärer Dissektion, Thermoplastik und lamellärer bandagierender Horse-Shoe Sklerokeratoplastik erreicht der Patient bei einer Nachbeobachtungszeit von 25 bzw. 14 Monaten bei stabiler Refraktion mit jetzt gut vertragenen Kontaktlinsen einen Visus von 0,8 rechts bzw. 0,5 links. Postoperative Komplikationen traten nicht auf.

Schlußfolgerung Zusammenfassend handelt es sich bei der lamellären Horseshoe Sklerokeratoplastik mit vorausgehender lamellärer Dissektion und Thermoplastik um eine technisch zwar schwierige, aber effiziente und komplikationsarme Methode zur Rehabilitation von Patienten mit peripherer Keratektasie.

Summary

Purpose Peripheral ectasia of the cornea is a rare, but rather complicated form of keratoconus, which in its severest form cannot be corrected by glasses or contact lenses. Penetrating corneal grafts must be positioned directly at the limbus. Such transplants mostly fail because of a high risk of immune reactions which can only be circumvented by systemic Ciclosporin A.

To avoid the potential risks of such a systemic immunomodulation we performed instead of a penetrating keratoplasty a special lamellar procedure which restored the normal configuration of the cornea.

Patient The 35-year-old man presented with best corrected visual acuity of 0.1 in both eyes. Contact lenses were not tolerated any longer.

Result After lamellar dissection, thermoplasty and lamellar horse-shoe sclerokeratoplasty the patient now wears well-fitting contact lenses and has a visual acuity of 0.8 with his right and 0.5 with his left eye after a follow-up period of 25 and 14 months respectively. There were no postoperative complications.

Conclusion Lamellar horse-shoe sclerokeratoplasty preceeded by lamellar corneal dissection and thermoplasty is a technically difficult, but efficient surgical measure to restore the normal corneal curvature in patients with advanced peripheral ectasia of the cornea.
