Spectral Optical Coherence Tomography Findings in Adult-Onset Foveomacular Vitelliform Dystrophy[1]

Zusammenfassung

Hintergrund: Die adulte vitelliforme Makuladystrophie (AVMD) ist typisch beidseitig und symmetrisch. Die Befunde der spektralen optischen Kohärenztomografie (OCT) von zwei Patienten mit einseitiger AVMD werden vorgestellt. Patienten und Methoden: Fall 1 ist ein 76-jähriger Mann, der während der letzten 6 Jahre wegen einer Makulaläsion in seinem rechten Auge angiografisch nachkontrolliert wurde. Sehschärfe rechts ist in dieser Zeit von 1,0 auf 0,6 gesunken. Fall 2 ist ein 69-jähriger Mann, der sich wegen verschwommenen Sehens rechts vorgestellt hat. Sehschärfe bei ihm war 0,7 rechts und 1,0 links. Bei beiden Patienten wurde eine Untersuchung mittels der spektralen OCT durchgeführt (Copernicus SOCT System). Ergebnisse: Bei dem ersten Fall zeigte die Fluoreszeinangiografie eine hyperfluoreszente juxtafoveale Läsion, die so groß wie 2 / 3 der Papillenfläche und ohne Spätleckage war. Die OCT zeigte eine zystische ovale Läsion zwischen dem retinalen Pigmentepithel (RPE) und den Photorezeptorschichten. Bei dem zweiten Fall zeigte die OCT eine hyperreflektive runde Läsion zwischen dem RPE und den Photorezeptorschichten mit Ablagerung des Lipofuscins. Schlussfolgerungen: Spektrale OCT erlaubt eine verbesserte Visualisierung der Netzhautschichten und liefert nützliche Informationen betreffend der Diagnose und der genauen Lokalisierung der AVMD.

Abstract

Background: Adult-onset foveomacular vitelliform dystrophy (AOFVD) is typically bilateral and symmetrical. We present the spectral optical coherence tomography (OCT) findings in two patients with unilateral AOFVD. Patients and Methods: Case 1 represents a 76-year-old man followed angiographically over the last 6 years for a macular lesion in his right eye. Visual acuity in the right eye has decreased from 1.0 to 0.6. In case 2, a 69-year-old man presented with blurred vision in his right eye. Visual acuity was 0.7 in the right eye and 1.0 in the left eye. In both patients, spectral OCT (Copernicus SOCT System) was performed. Results: In case 1, fluorescein angiography showed a juxtafoveal hyperfluorescent lesion 2 / 3 of disc area in size, without late leakage. OCT showed a cystic oval lesion with thick walls, located between the retinal pigment epithelium (RPE) and photoreceptor layers. In case 2, OCT showed a hyperreflective round lesion also located between the RPE and photoreceptor layers, elevating the overlying inner retina. Conclusions: Spectral OCT enables enhanced visualization of the retinal layers and provides useful information regarding the diagnosis and the exact location of AOFVD.
