Schmerztherapie in der Palliation von Krebskranken - Praktische Aspekte zur Lösung von Behandlungsproblemen

Etwa ein Viertel der Tumorpatienten in Deutschland klagen schon bei der Diagnosestellung über Schmerzen. Für eine effektive Schmerztherapie ist eine Schmerzklassifizierung - nozizeptiver Schmerz, neuropathischer Schmerz, mixed pain - unabdingbar. Empfehlungen und Leitlinien zur Schmerztherapie ermöglichen eine gute schmerztherapeutische Versorgung von Tumorpatienten. Bereits in der frühen Phase einer malignen Erkrankung empfiehlt es sich, mit dem Patienten über das gewünschte Procedere bei eventuell eintretender Krankheitsprogression zu sprechen. Dabei sind Basiskenntnisse in der Kommunikation mit Sterbenden und Schwerstkranken unerlässlich. Eine Patientenverfügung hilft den Patienten, ihren Willen unmissverständlich zu dokumentieren.

Approximately every fourth German cancer parient complains of pain already during diagnosis. Effective pain therapy requires first of all a clear differentiation between the various types of pain: nociceptive, neuropathic, and mixed pain. Recommendations and pain therapy guidelines facilitate a good therapeutic approach. It is advisable to discuss the desired procedure with the patient already in the early phase of a malignant disease while bearing in mind that the disease may become increasingly serious and painful. In this regard it is imperative that the physician commands over sufficient knowledge of how to communicate with fatally ill patients and with those who lie dying. If the patient deposits a valid written declaration of intent he can communicate his intentions clearly and unequivocally.
