Summary
We observed an isolated olfactoric-gustatory aura continua in the course of a probable Herpes-simplex virus (HSV) encephalitis. From the beginning of the illness, the patient noticed a continuous dysosmia in the left nostril accompanied by a gustatory dysaesthesia which both continued for 14 days.
The diagnosis was based on a reproducible effect of midazolam tested against placebo and the occurrence of one secondary generalised seizure. The dysosmia was present only in the left nostril, ipsilateral to a constant temporal theta focus in the surface EEG. This indicates epileptic activity generated from mesiobasal limbic structures of the left temporal lobe. We suggest that the sporadically reported olfactoric-gustatoric hallucinations during HSV-encephalitis could correspond with simple partial seizures.
Zusammenfassung
Wir beobachteten eine isolierte olfaktorisch-gustatorische Aura continua bei viraler, wahrscheinlich durch Herpes-simplex-Virus (HSV) verursachter Enzephalitis. Die Diagnose stützte sich auf eine reproduzierbare Unterbrechung der Symptomatik durch Midazolam bei Wirkungslosigkeit von Plazebo und die Beobachtung eines sekundär generalisierten epileptischen Anfalls.
Die olfaktorische Mißempfindung wurde ausschließlich links und somit ipsilateral zu einem temporalen Thetafokus im EEG wahrgenommen. Dies weist auf eine links mediobasal limbische Generation epileptischer Aktivität hin. Wir stellen zur Diskussion, ob olfaktorisch-gustatorische Halluzinationen bei HSV-Enzephalitis pathophysiologist als elementar-fokale Anfälle aufzufassen sind.
