Liquorzytologische Befunde bei HIV-infizierten Patienten mit neurologischen Symptomen

Summary

Cerebrospinal fluid (CSF) cytology and immunocytological studies were performed in 137 HIV-infected patients in stages WR3-WR6 with different neurologic symptoms. According to clinical, radiological and autoptic findings a diagnostic distribution in the groups of chronic HIV encephalitis (n = 80), opportunistic infections of the central nervous system with toxoplasmosis (n = 4), cytomegalovirus (n = 5), papovavirus (n = 4), cryptococcus (n = 1), primary CNS lymphoma (n = 2) and secondary CNS lymphoma (n = 5) was made. The CSF cytological examination, including immunocytology, was very helpful in diagnosing cryptococcal meningitis and lymphomatose meningeal involvement. Patients with chronic HIV encephalitis presented with an increase of immunodeficiency a decrease in cell count. Bleocytosis with granulocytes indicated an opportunistic CNS-infection, except for progressive multifocal leukencephalopathy, which was characterised by very low cell counts.

Zusammenfassung

Liquorzytologische und immunzytologische Untersuchungen wurden bei 137 HIV-infizierten Patienten aus den Stadien WR3-WR6 mit verschiedenen neurologischen Symptomen durchgeführt. Aufgrund klinischer, radiologischer und autoptischer Befunde wurde eine diagnostische Zuordnung in die Gruppen chronische HIV-Enzephalitis (n = 80), opportunistische Infektionen des zentralen Nervensystems (ZNS) mit Toxoplasmose (n = 40), Zytomegalie (n = 5), Papovavirus (n = 4), Kryptokokkose (n = 1), primäre ZNS-Lymphome (n = 2) und sekundäre ZNS-Lymphome (n = 5) vorgenommen. Die Liquorzytologie, einschließlich Immunzytologie, erwies sich in der Diagnostik der Kryptokokkenmeningitis und der Meningiosis lymphomatosa als sehr hilfreich. Patienten mit chronischer HIV-Enzephalitis zeigten mit zunehmendem Immundefekt einen Abfall der Zellzahl. Eine Zellzahlerhöhung mit Granulozyten im Zellbild wies auf eine opportunistische ZNS-Infektion mit Ausnahme der progressiven multifokalen Leukenzephalopathie hin, die sich durch sehr niedrige Zellzahlen auszeichnete.
