Zusammenfassung
Im klinischen Alltag der Neurologie spielen Kollagenkrankheiten und Vaskulitiden nur
eine zahlenmäßig untergeordnete Rolle. Es ist jedoch wichtig, in der Differenzialdiagnose
an sie zu denken. Für zerebrale Manifestationen dieser Erkrankungen ist die Kombination
von Kopfschmerzen, multifokalen Ausfällen und einer Enzephalopathie häufig: Leitsymptom
der peripher neurologischen Manifestationen ist das Bild der subakuten, oft schmerzhaften
Mononeuropathia multiplex. Wegweisend in der Diagnostik sind bildgebende Verfahren,
Liquor- und Serumbefunde. Stets ist die histologische Absicherung der Diagnose erforderlich.
Summary
Collagen vascular diseases and vasculitides are infrequent in neurological practice
but important for the differential diagnosis of both CNS and PNS syndromes. The combination
of chronic headache, multiple strokes and encephalopathy is the most frequent CNS
manifestation of vasculitis. Peripheral nervous system symptoms in rheumatic diseases
include compression syndromes and mononeuritis multiplex in vasculitis. Besides MRI,
angiography, CSF and serum findings, bioptic verification of the diagnosis is mandatory
in these disorders.
