Zusammenfassung
Eine 24-jährige Studentin leidet an rezidivierenden abdominellen Beschwerden, nicht kolikartig, sondern dumpf, ähnlich wie bei einem Schlag ins Abdomen vor ca. 3 Monaten. In der laborchemischen Untersuchung fällt ein erhöhter Lipasewert von 1 300 U/l (Norm < 100 U/L) auf, ohne dass weitere pathologische Abweichungen bestehen. Ein größerer Alkoholkonsum wird glaubhaft verneint. Die gesamte bildmorphologische Diagnostik deutet auf ein fortgeschrittenes Pankreaskarzinom hin, lediglich der anamnestische Hinweis auf den Schlag ins Abdomen lässt auf die Differenzialdiagnose eines Pankreastraumas hindeuten, die in einer histologischen Untersuchung des Punktates bestätigt werden kann. Das weitere konservative Procedere führt schließlich zu einer restitutio ad integrum des Pankreas.
Abstract
A 24-year-old woman is suffering an abdominal pain, not like a colic but dull like after a blow in the abdomen 3 months ago. Her laboratory data show an increased lipase 1 300 U/l (norm < 100 U/l) without any other pathological changes. An increased alcohol consumption is denied. All images raised the suspect of an advanced tumor of the pancreas, only the advice in her history of an abdominal trauma 3 months ago raise a suspect of a possible panceas trauma. The following histological examination confirmed the diagnosis of a possible trauma of the pancreas. Subsequently a konservative treatment regime lead to a successful restitutio ad integrum of the pancreas.
