Update zur medikamentösen Therapie des idiopathischen Parkinson-Syndroms (IPS)

Diese Übersicht diskutiert die wesentlichen pharmakotherapeutischen Innovationen und Studien der letzten zwei Jahre bei idiopathischem Parkinson-Syndrom (IPS). Neuigkeiten gibt es zu dem weiterhin wichtigen L-DOPA, eine Helicobacter-pylori-Eradikation soll die L-DOPA-Resorption erleichtern, zu den ohne L-DOPA eine Rationale entbehrenden COMT-Hemmer Entacapon und Tolcapon sowie zu Rivastigmin, dem ersten zugelassenen Cholinesterasehemmer für die Indikation Demenz bei IPS. Der zwanghafte Gebrauch von Dopaminergika wird mit neuen Begriffen wie die hedonistische homeostatische Dysregulation (HHD) und Dopamindysregulationssyndrom (DDS) problematisiert. Neu verfügbar sind der MAO-B-Hemmer Rasagilin, der neue Non-Ergot-Dopaminagonist und einziges Parkinsonmedikament in Pflasterform Rotigotin, die Pumpenmedikamente für die subkutane Dauerinfusion von Apomorphin und L-DOPA/Carbidopa-Gel für die kontinuierliche jejunale Darreichung über eine PEG.

The review covers the main innovations and studies in pharmacotherapy for Parkinson's disease (PD) during the last two years: levodopa, entacapone in combination with levodopa and carbidopa, the MAO-B inhibitor rasagaline, and the new non-ergot dopamine agonist and only antiparkinsonian drug for transdermal delivery rotigotine, apomorphine for continuous subcutaneous and levodopa/carbidopa gel for jejunal delivery, rivastigmine the only licensed cholinesterase inhibitor for the indication dementia associated with PD.
