Large soft tissue defect and bone defect caused by trauma are serious problems in orthopedic trauma. Conventional flaps are used in simple cases, and large soft tissue defects and septic defects are covered by microsurgical tissue transplantation.

The authors presented 20 cases treated with microsusrgical tissue transplantation: 14 cases of soft tissue transplantation, 4 cases of bone transplantation, and 2 cases of bone and soft tissue transplantation. Soft tissue coverage was performed in ten cases by latissimus dorsi free flaps and in 4 cases by Chinese flaps. Free fibula transplantation was performed in 4 cases, iliac crest transplantation in one case, and musculocutaneous fibula transplantation in another case. Fifteen cases were septic; in 10 cases, osteomyelitis was present. All patients healed without late complications, and complete bony healing was achieved.

Microsurgical tissue transplantation is the best method for covering large and/or septic soft tissue defects, and for reconstruction of bone defects and osteomyelitis.
