Reconstruction of maxillary bone loss of substances must achieve aesthetic and functional results: 1. restoring zygomatic bulking and symmetry of the middle third of the face; 2. reconstruction of the orbital floor giving a correct support to the bulbus ocularis anad 3. reconstruction of a maxillary bone adequate enough to receive dental implants.

The free fibular flap is widely used as a first choice reconstructive technique for large maxillary defects. The use of this flap allows three–dimensional reconstruction of bone segments and soft tissue, restoring maxillary functional and aesthetical defects.
