Zusammenfassung

Hintergrund & Zielsetzung: Der diagnostische Benefit der Kapselendoskopie bei Untersuchungen des Dünndarms ist hoch und den konventionellen Methoden und szintigraphischen Verfahren überlegen. Aufgrund unzureichender klinischer Erfahrung raten die amerikanische Food & Drug Administration (FDA) und der Hersteller Given Imaging GmbH von einer Durchführung der Kapselendoskopie bei Patienten mit Herzschrittmachern und anderen elektrischen Implantaten ab. Wir untersuchten die möglichen Interferenzen zwischen der Kapselendoskopie und einem Herzschrittmacher in vitro. Methode: Eine Kapsel wurde in einen gereinigten Schweinedarm eingebracht. Der Darm befand sich in horizontaler Ausrichtung in einem mit 5 %iger Glukoselösung gefüllten Plastikbeutel. Die gesendeten Signale der Kapsel wurden mittels acht Sensoren zum Datenrekorder übermittelt. Einen Schrittmacher (DDD-Modus) platzierten wir in der unmittelbaren Nähe des Darms/der Kapsel auf der zu den Elektroden kontralateralen Seite. Die Funktion des Herzschrittmachers wurde permanent überwacht. Abschließend platzierten wir die Elektrode uni- bzw. bipolare Stimulation (VVI-Modus), in die Glukoselösung in unmittelbarer Nähe des Darms und leiteten permanent über 10 Minuten ein EKG ab. Ergebnisse: Zwischen der Kapsel und dem Herzschrittmacher traten trotz des geringen Abstandes keine Interferenzen auf. Schlussfolgerung: Die Kapselendoskopie erscheint bei Patienten mit Herzschrittmachern vertretbar.

Abstract

Background & Aims: The diagnostic benefit of capsule endoscopy (CE) in suspected small bowel disease is high and seems to be superior to conventional methods and scintiscan. Because of the few clinical experiences CE should not be used in patients with cardiac pacemakers and other implanted electrical devices (recommendation of FDA and Given Imaging). We analysed potential interferences between CE and a cardiac pacemaker in vitro. Methods: A capsule was placed in a cleaned gut of a pork. The gut was inserted in a horizontal direction into a plastic bag filled with a glucose solution. With eight sensors the transmitting signals of CE were sent to a receiving unit. A cardiac pacemaker (DDD-Mode) was placed in proximity of the gut/CE opposite to the electrodes. The function of the pacemaker was observed permanently. Finally an electrode with the cardiac pacemaker in VVI-Mode (uni- and bipolar stimulation), was placed in the glucose solution near the gut. An ECG was recorded for about 10 minutes. Results: There were no interferences between the CE and the cardiac pacemaker in our experiment in spite of the quite nearness of CE and cardiac pacemaker. Conclusion: The clinical use of CE seems to be unproblematic in patients with cardiac pacemakers.
