Zusammenfassung

Die Diagnose einer Appendizitis ist bei Kindern schwierig und daher die Rate an Perforationen mit 50-70 % extrem hoch. Bei unklarer Diagnose kann im Rahmen einer diagnostischen Laparoskopie die simultane Appendektomie beim leisesten Verdacht auf eine Appendizitis ohne Schwierigkeiten und zum Wohl der Kinder vorgenommen werden. Seit 1988 haben wir in unserer Tagesklinik neben anderen laparoskopischen Operationen einschließlich laparoskopischer Appendektomien (LA) bei Erwachsenen und Schwangeren diesen Eingriff auch bei 22 Kindern durchgeführt. Alle Kinder wurden von Familienangehörigen während des eintägigen Aufenthaltes in unserer Einrichtung begleitet und verließen diese am späten Nachmittag oder frühen Abend. Postoperativ traten bei zwei Mädchen Komplikationen auf, welche eine stationäre Behandlung erforderlich machten. Die histomorphologische Untersuchung der Appendizes zeigte eine chronisch-rezidivierende Entzündung in 9 Fällen und eine akute, subakute und frisch-phlegmonöse oder ulzeröse Appendizitis in 13 Fällen. Die ambulante laparoskopische Appendektomie ist eine mögliche Ergänzung der konventionellen pädiatrischen Chirurgie.

Abstract

As the diagnosis of appendicitis is particularly difficult in infants, one can decide in favour of a diagnostic laparoscopy with simultaneous appendectomy in the slightest suspicion of appendicitis to decrease the high rate of 50-70 % of appendix perforation in children, as a benefit for them. Since 1988 we performed outpatient laparoscopic appendectomy successfully in adults and pregnant women as we did in 22 infants. All children were accompanied by family-members during their stay in our institute and were discharged in the late afternoon or early evening of the same day. Complications occurred in two girls postoperatively who were cured in hospital. The histomorphical outcome of the removed appendices was chronic recurrent infection in 9 cases and acute, sub-acute and fresh-phlegmonous or ulcerous appendicitis in 13 cases. Outpatient laparoscopic appendectomy is a valuable method in pediatric sugery.
