Zusammenfassung
Hintergrund: Die DEGAM-Leitlinie Nr. 2 Müdigkeit steht nach Ablauf des „Verfallsdatums” zur Überarbeitung an. Methoden: Wir sichteten die Literatur seit 2001 erneut nach den Schlagwörtern fatigue und general practice und führten zudem gezielte Recherchen zu kontroversen Punkten und verwandten Leitlinien durch. Ergebnisse: Im Wesentlichen wurden die Aussagen der bisherigen Leitlinie bestätigt: Depression ist die häufigste Ursache primär unklarer Müdigkeit. Kognitiv-behaviorale Verfahren und körperliche Aktivierung sind die am besten belegten Therapieverfahren bei diesem Symptom. In einigen Punkten waren Ergänzungen und leichte Modifikationen erforderlich, die genauer beschrieben werden. Schlussfolgerung: Der Beratungsanlass Müdigkeit stellt weiterhin eine Herausforderung für den Hausarzt dar. Vertiefte Kenntnisse über die psychosomatische Grundversorgung sind dabei hilfreich und unentbehrlich für eine qualitativ gute Patientenversorgung.
Abstract
Background: The guideline fatigue of the German Scientific Association of General Practice/Family medicine (DEGAM) needs updating. Methods: We screened for literature with the terms fatigue and general practice since 2001 and searched in depth for controversal subjects and similar guidelines. Results: The key-messages of the present guideline were generally confirmed. Depression is the most important diagnosis to consider in patients presenting with fatigue. The interventions with the best available evidence were cognitive-behavioral-therapy and physical activation. Only slight changes and additions had to be made which are described in the article. Conclusion: Fatigue as reason for encounter is still a challenge for General Practicioners/Family Medicine. Well developed communication skills and a thorough understanding of psychosomatic relationships are required in the care for this group of patients.
