Unspezifische Beschwerden als Beratungsergebnis in einer Allgemeinarztpraxis

Zusammenfassung
Hintergrund: Beratungsergebnisse lassen sich im derzeitigen Abrechnungssystem nicht valide mittels der Routine-Daten erfassen, da die „Krankenscheindiagnosen” Abrechnungsbegründungen darstellen, aber nicht das tatsächliche Geschehen in der Hausarztpraxis abbilden. Methoden: Im Rahmen einer Praxisstudie einer großen ländlichen Allgemeinarztpraxis wird der Anteil unspezifischer Beschwerden an der Gesamtzahl der Beratungsergebnisse des Jahres 2005 (n = 12 861) ermittelt. Ergebnisse: Unspezifische Beschwerden als Beratungsergebnis sind mit 6,6 % relativ gering aber regelmäßig anzutreffen. Durch gezieltes Befragen lassen sich 70 % dieser Beschwerden individuellen oder sozialen Problemen der Patienten zuordnen. Schlussfolgerung: Beratungsergebnisse, die sich nicht primär einem Fachgebiet zuordnen lassen, sind in unserer Praxis selten und dann meistens durch psychosoziale Belastungen erklärbar. Für die restlichen 2 % der Beratungsergebnisse empfiehlt sich eine programmierte/strukturierte Diagnostik, um abwendbar gefährliche Verläufe zu erkennen.
Abstract
Background: Results of consultation cannot be easily obtained in General Practice in Germany, because routine data do not reflect the real situation in practice - they are used as reasons for accounts. Methods: In a large rural general practice, the total number of consulting conclusions of the year 2005 (n = 12 861) was analysed. Persons with organic symptoms, which could be assigned to special clinical pictures are compared with those having only unspecific symptoms. Results: Within the relatively low percentage of persons with unspecific symptoms (6.6 %) most patients had psychosocial problems (70 %). Conclusion: About 2 % of the consultations could not be analysed by common methods and should be examined by programmed or structured diagnosis in order to explore the cause of their complaints and to avert a dangerous development
