Zusammenfassung

Hintergrund: Bei der myotonen Dystrophie Curschmann-Steinert handelt es sich um eine multisystemische, autosomal dominant vererbte Erkrankung, welche durch eine distal betonte Muskelschwäche, Muskelatrophie, Myotonie und Facies myopathica charakterisiert ist. Eine okuläre Beteiligung manifestiert sich meist in Form von Katarakt, Ptosis und intraokularer Hypotension. Im Krankheitsverlauf kommen oft Herzmuskelbeteiligung, endokrine Störungen, Ateminsuffizienz infolge der Schwäche der respiratorischen Muskulatur und gastrointestinale Beschwerden hinzu. Die potenzielle Symptomvielfalt und die variable klinische Ausprägung führen dazu, dass das Krankheitsbild häufig übersehen wird. Kasuistik: Eine 58-jährige Patientin wurde uns im Januar 2006 mit einer beidseitigen, langsam zunehmenden Visusminderung, Rückenverspannungen und -schmerzen sowie einer progredienten Schwäche des rechten und geringer auch des linken Beins überwiesen. Die ophthalmologische Beurteilung ergab eine sternförmige hintere subkapsuläre Katarakt, eine leichte beidseitige Ptosis und einen abgeschwächten Lidschluss. Auffällig am Gesamthabitus der Patientin waren ein „müder Gesichtsausdruck” im Sinne einer „Facies myopathica” und eine mäßige Kachexie. Neurologisch bestand eine Muskelschwäche und -atrophie der Extremitätenmuskulatur sowie der Musculi sternocleidomastoidei und temporales, eine leichte Dysarthrophonie und Gangunsicherheit. Laborchemisch war eine nur mäßige Kreatinkinase(CK)-Erhöhung nachweisbar. In der Elektromyographie (EMG) ließen sich Zeichen eines myopathischen Prozesses in der proximalen Muskulatur der unteren Extremität (M. vastus medialis rechts und links) nachweisen. Obwohl im EMG keine sicheren, myotonen Entladungsserien festgestellt wurden, bestätigte die molekulargenetische Untersuchung die klinische Verdachtsdiagnose einer myotonen Dystrophie Curschmann-Steinert. Schlussfolgerung: Die Kombination einer myotonen Muskelerkrankung mit ophthalmologischen und gegebenenfalls kardialen, abdominellen Symptomen, respiratorischer Insuffizienz und Schlaf-Apnoe-Syndrom sollte den Verdacht auf eine myotone Dystrophie lenken. Aufgrund der Häufigkeit okulärer Manifestationen, insbesondere der myotonen Katarakt, kann eine augenärztliche Beurteilung wertvolle Hilfe bei der Diagnosestellung leisten.

Abstract

Background: Myotonic dystrophy Curschmann-Steinert is a multisystemic disorder inherited as an autosomal dominant trait. Characteristic clinical findings are muscular weakness and distal atrophy of the extremities, myotonia and myopathic facies. Ophthalmological symptoms may include cataract, ptosis and intraocular hypotension. Cardiac conduction defects, endocrine disturbances, respiratory insufficiency and gastrointestinal affection appear quite often in the course of the disease. Because of the great diversity of symptoms and the variable clinical presentation the disease may be overlooked. Case Report: In January 2006, a 58-year-old woman presented with progressive vision loss on both eyes, lumbal discomfort and pain, as well as a gradual weakness of the right leg. Ophthalmological evaluation revealed a star-shaped posterior subcapsular cataract, subtle ptosis on both eyes and weakness of the eyelid closure. Morphologically we observed a lack of facial expression manifesting as a myopathic facies and a moderate cachexia. Neurological findings included muscle weakness and atrophy in the extremities as well as of the sternocleidomastoid and temporal muscles a slight dysarthrophony and gait disturbance. Furthermore, an increase of the creatine kinase levels in the blood was detected. Electromyography (EMG) showed signs of a myopathic process in the proximal muscles of the lower extremities (right and left vastus medialis muscle). Although no typical myotonic discharges were recorded in the EMG, molecular testing confirmed the clinical suspicion of myotonic dystrophy type Curschmann-Steinert. Conclusions: The combination of a myotonic muscle disorder with ophthalmological, and probably cardiac, abdominal and pulmonary symptoms, especially respiratory insufficiency and sleep apnoea should lead to the suspicion of a myotonic dystrophy. Due to the high incidence of ocular manifestations, especially of the typical myotonic cataract, an ophthalmological investigation is essential for a straightforward diagnostic procedure.
