Definitionen, Klinik und Ätiopathogenese

Die funktionelle Dyspepsie beschreibt ein Krankheitsbild, das ca. 50 % der Patienten betrifft, die sich mit dyspeptischen Beschwerden (frühzeitiges Sättigungsgefühl, postprandiales Völlegefühl, epigastrischer Schmerz, Übelkeit, Aufstoßen, Sodbrennen, Aufgeblähtsein etc.) in der ärztlichen Praxis vorstellen.

Bei der funktionellen Dyspepsie handelt es sich um eine Ausschlussdiagnose. Es sollte eine klare begriffliche Trennung zwischen „nicht abgeklärten” dyspeptischen Beschwerden und „abgeklärter” funktioneller Dyspepsie eingehalten werden.

Die Erkrankung hat einen unklaren molekularen Pathomechanismus, aber Störungen im autonomen Nervensystem und der zentralen Verarbeitung von Reizen aus dem enterischen Nervensystem (viszerale Hypersensibilität) werden als Ursache für die funktionelle Dyspepsie angenommen.

Diagnostik und Therapie

Das Management dyspeptischer Beschwerden ist von Patientenfaktoren (Alter, Alarmsymptome), Anamnese, Dauer und Intensität der Beschwerden sowie Kostenstrukturen innerhalb eines Gesundheitssystems abhängig. Empirische Therapie über 4 - 6 Wochen, gezielte Endoskopie und Therapie, Hp-Testung und Eradikation oder Säure supprimierende Therapie kommen als Alternativen in Frage.

Bei nachgewiesener funktioneller Dyspepsie spielen ein stabiles Vertrauensverhältnis zwischen Arzt und Patient, die Aufklärung des Patienten und die Entwicklung eines Krankheitsmodells als Erklärung für die Beschwerden eine zentrale therapeutische Rolle.

Bei einer möglichen medikamentösen Therapie gibt es verschiedene Alternativen, aber die Wirkung der Mehrzahl der Präparate liegt nur 10 - 20 % über der in den meisten Studien relativ hohen Plazeboheilungsrate von ca. 40 %. Unter diesen Behandlungsalternativen gibt es keinen „Goldstandard”, so dass ggf. verschiedene Therapieansätze versucht werden können und müssen.

Bei den derzeit verwendeten Therapieansätzen handelt es sich um motilitätsfördernde/oder -modulierende Pharmaka (Prokinetika), Säure supprimierende Medikamente (H2-Blocker, PPI), die Hp-Eradikation und alternative Ansätze wie Phytotherapeutika oder psychologische Behandlungsansätze.
