Zusammenfassung
Die häufigste Herausforderung in der Parkinson-Therapie mit Levodopa stellen motorische
Komplikationen dar. Sie können gegen Ende eines Levodopadosierungsintervalls als so
genanntes Wearing-off oder im weiteren Verlauf als unvorhersehbare On-off-Fluktuationen
auftreten. Zu diesen Komplikationen kommt es oft unter langjähriger Therapie mit Standard-Levodopa,
sie können jedoch auch schon nach einigen Monaten vorkommen. Zur Therapie eignet sich
die Kombination von Levodopa, Carbidopa und Entacapon (Stalevo®), die die Wearing-off-Symptome
und Off-Zeiten wirksam vermindern kann. Das vorliegende Fallbeispiel zeigt einen Patienten,
der nach nur zweijähriger Therapie mit Standard-Levodopa unter starken motorischen
und nichtmotorischen Symptomen litt. Bei Umstellung auf Stalevo® besserten sich diese
deutlich. Die Gabe der drei Wirkstoffe in einer Tablette vereinfacht die Medikamenteneinnahme
erheblich.
Abstract
The most common challenge in the treatment of Parkinson's disease with levodopa are
motor complications. They can emerge at the end of an levodopa-dosing-interval as
so-called wearing-off or in the progress of the disease as unpredictable on-off-fluctuations.
This complications often occur in long lasting therapy with standard-levodopa, but
they can also appear after only some months of treatment. Suitable for the treatment
of fluctuations is the combination of levodopa, carbidopa and entacapone (Stalevo®),
which can reduce wearing-off and off-time effectively. The present case report shows
a patient who suffered of severe motor and non-motor symptoms after only two years
of treatment with standard-levodopa. With the change of medication to Stalevo® those
improved markedly. Prescription of the three active agents in one tablet considerably
facilitates drug intake.
