Zusammenfassung

Die Ballondilatation von Crohn-Strikturen spielt eine große Rolle im Management dieser chronischen Erkrankung. Bei dominanten Strikturen, die endoskopisch erreichbar und kurzstreckig sind, ist die endoskopische Dilatation die Therapie der ersten Wahl. Die Patienten sollten über das geringe aber vorhandene Risiko des Eingriffs aufgeklärt werden, die Möglichkeit wiederholter Eingriffe und die eventuelle Notwendigkeit eines chirurgischen Eingriffs trotz initial erfolgreicher Endoskopie muss deutlich gemacht werden. Eine prätherapeutische röntgenologische Beurteilung, eine sorgfältige Patientenselektion, ein erfahrener Endoskopiker und die Nutzung adäquaten Equipments sind essenzielle Voraussetzungen für einen sicheren und erfolgreichen Eingriff.

Abstract

Balloon dilation of Crohn's strictures plays an important part in the management of this chronic disease. The authors conclude endoscopic dilation should be considered as a first-line therapy for short, accessible, dominant strictures. Patients undergoing dilation should be aware of the small but potenzially serious risks of the procedure, the likely need for repeat procedures and the possibility that surgery may still be necessary despite initial endoscopic success. Pre-procedure radiological assessment, careful patient selection, an experienced endoscopist, and access to the appropriate equipment and facilities are all essential requirements for a safe and successful procedure.
