Zusammenfassung

Für die Therapie der Migräne haben sich in jüngster Zeit einige neue Aspekte ergeben. Ein wesentlicher Aspekt ist die Veränderung des Spektrums der für die Behandlung der Migräne zur Verfügung stehenden Präparate. Obwohl in den letzten Monaten fast alle Ergotaminpräparate vom Markt genommen wurden, stehen neben den herkömmlichen peripheren Analgetika mittlerweile sieben verschiedene Triptane (5-HT1B/D-Agonisten) in zahlreichen Applikationsformen zur Verfügung, sodass auch weiterhin ein reichhaltiges Medikamentenarsenal für die Akuttherapie existiert. Als neueste Generation von Migräne-Akuttherapeutika sind CGRP-Antagonisten erstmals erfolgreich in klinischen Studien getestet worden. Aber auch für die Prophylaxe der Migräne haben sich zahlreiche Änderungen ergeben. Mehrere seit Jahren in der Prophylaxe der Migräne zur Verfügung stehende Präparate haben ihre Zulassung verloren. Neue Substanzen wie zum Beispiel Topiramat konnten hingegen erfolgreich in der Prophylaxe der Migräne getestet werden und stehen vor der Zulassung. Dieser Artikel fasst die wesentlichen Aspekte sowie die neuesten Erkenntnisse der Akuttherapie und Prophylaxe der Migräne zusammen.

Summary

Within the past few months several changes for the treatment of headache and migraine came up. One of the main modifications is the change of available drugs for the treatment of migraine. Despite the fact that almost all ergot derivatives have been taken off the market, seven triptans (5-HT1B/D-agonists) are now available in several different preparations assuring a comprehensive arsenal of anti-migraine drugs. As a completely new generation, the first CGRP-antagonist has been successfully tested in clinical trials for the treatment of acute attacks. As for the prophylactic treatment of migraine several changes occurred here as well. Again, several agents that have been introduced decades ago for the prophylaxis of migraine have been taken off the market. On the other hand new agents such as topiramate have been successfully tested for the prophylaxis of migraine and are about to be approved in many countries. The following review will summarize the key aspects and the newest development in the acute and prophylactic treatment of migraine.
