Zusammenfassung

Bei etwa 20 % aller Patienten mit ischämischem Schlaganfall lassen sich kardiale Emboliequellen nachweisen, bei 40 % der Patienten liegen Risikofaktoren für die Entstehung kardialer Embolien vor. Daher sollten alle Patienten nach ischämischem Schlaganfall kardiologisch evaluiert werden. Anamnese, körperliche Untersuchung, ein 12-Kanal-EKG und eine transthorakale Echokardiographie (Schnittbildtechnik, Doppler-Echokardiographie und Farb-Dopplerechokardiographie) gehören zur Basisdiagnostik bei jedem Patienten. Darüber hinaus sollten ein Langzeit-EKG und eine 24-Stunden Blutdruckmessung (ABDM) durchgeführt werden. Zum Ausschluss eines intrakardialen Shunts sollte zusätzlich eine Kontrastechokardiographie in Ruhe und unter Valsalva-Manöver durchgeführt werden. Eine transösophageale Echokardiographie sollte zusätzlich bei pathologischem transthorakalen Befund, bei klinischem Verdacht auf eine Endokarditis, bei suboptimalen transthorakalen Untersuchungsbedingungen, bei unklarem klinischen Status sowie bei transthorakalem Nachweis eines intrakardialen Re-Li-Shunts erfolgen.

Summary

Intracardiac thrombi or mobile valvular vegetations can be found in 20 % of patients with ischemic stroke. Risk factors for cardiac emboli are present in about 40 % of these patients. Thus, all patients with ischemic stroke should have complete cardiological evaluation. Physical examination, 12-lead electrocardiogram and transthoracic echocardiography (including cardiac imaging, Doppler echocardiography as well as color flow mapping) should be performed in every patient after ischemic stroke. Furthermore Holter monitoring and ambulatory blood pressure monitoring (24 hours) should be performed. Screening for intracardiac shunts should performed by contrast transthoracic echocardiography (during quiet breathing and after Valsalva maneuver). Patients should be evaluated by transesophageal echocardiography additionally in case of pathologic findings during transthoracic echocardiography, in patients with clinical suspicion of endocarditis, in case of nondiagnostic clinical status, in patients with suboptimal imaging quality during transthoracic echocardiography and when an intracardiac shunt has been documented by transthoracic contrast echocardiography.
