Summary

Gastrointestinal symptoms may have banal causes, but may also arise in a life-threatening condition. The leading symptom for oesophageal diseases is dysphagia. The most common cause of dysphagia in patients older than 45 years is oesophageal dysphagia - in younger patients reflux disease and motility disorders.

Acute gastritis develops either as a result of exogenous noxae (excessive alcohol, ASA, NSAIDs) or stress. Symptoms include loss of appetite, nausea, vomiting, eructation, a sensation of fullness in the upper abdomen, and an unpleasant taste in the mouth. In the majority of cases, chronic gastritis does not give rise to symptoms.

The most common cause of ulcer disease is Helicobacter pylori infection, and the most common symptom is epigastric pain. A whole range of symptoms in the region of the upper abdomen is subsumed in the term dyspeptic syndrome. Possible causes are a change in nociception and psychosomatic factors. In the case of an upper gastrointestinal bleed, the source of the bleeding is proximal to the ligament of Treitz, that is in the oesophagus, stomach or duodenum. Clinical manifestations are haematemesis or tarry stools.
