Zusammenfassung

Hintergrund: Zweck dieser Studie war es, die Wirksamkeit der selektiven Lasertrabekuloplastik (SLT) bei der Behandlung von primärem Offenwinkelglaukom (POWG) in einer prospektiven klinischen Studie zu erforschen. Patienten und Methoden: 36 Augen von 36 Patienten mit unkontrolliertem POWG wurden mit dem gütegeschalteten frequenzverdoppelten Nd:YAG-Laser (532 nm) behandelt. Der Intraokulardruck (IOD) wurde vor der Behandlung gemessen sowie einen Tag, eine Woche, einen Monat und 3, 6, 12, 18, 24, 30, 36, 42 und 48 Monate danach. Misserfolg wurde als eine IOD-Senkung unter 20 % des Vorbehandlungs-IOD definiert oder als eine Gesichtsfeldverschlechterung oder Progression der Sehnervenkopfschädigung, welche die Filtrationschirurgie verlangte. Die IOD-senkende medikamentöse Therapie wurde während dieser Studienzeit nicht modifiziert. Ergebnisse: Die durchschnittliche Nachbeobachtungszeit betrug 34 Monate (SD 12,9). Der durchschnittliche Vorbehandlungs-IOD betrug 22,9 mm Hg (SD 2,1). Nach einmonatiger Nachbeobachtungszeit betrug die durchschnittliche IOD-Senkung 5,3 mm Hg (SD 2,1) oder 23,1 % und nach 6 Monaten 5,6 mm Hg (SD 2,6) oder 24,5 %. Nach 12-monatiger Nachbeobachtungszeit betrug die durchschnittliche IOD-Senkung 5,6 mm Hg (SD 2,3) oder 24,5 % und nach 24 Monaten 6,2 mm Hg (SD 2,5) oder 27 %. Nach 36-monatiger Nachbeobachtungszeit betrug die durchschnittliche IOD-Senkung 6,4 mm Hg (SD 2,1) oder 27,4 % und nach Ende der 48-monatigen Nachbeobachtungszeit betrug die durchschnittliche IOD-Senkung 5,9 mm Hg (SD 2,0) oder 25,4 %. Nach der Kaplan-Meier-Überlebensanalyse betrug die 12-monatige Erfolgsziffer 97 %, die 24-monatige Erfolgsziffer 88 %, die 36-monatige Erfolgsziffer 76 % und die 48-monatige Erfolgsziffer 71 %. Schlussfolgerung: Die SLT ist eine wirksame Methode bei der Behandlung von POWG.

Abstract

Background: Our aim was to investigate the efficacy of selective laser trabeculoplasty (SLT) for the treatment of primary open-angle glaucoma (POAG) in a prospective clinical study. Patients and methods: In 36 eyes of 36 patients suffering from uncontrolled POAG, treatment was carried out with a frequency-doubled, Q-switched Nd:YAG laser (532 nm). The intraocular pressure (IOP) was measured before the treatment and one day, one week, one month and 3, 6, 12, 18, 24, 30, 36, 42 and 48 months after. A failure was defined as an IOP reduction of less than 20 % from pretreatment IOP, or a progression of visual field or optic disc damage requiring filtering surgery. The hypotensive medication during the study period remained unchanged. Results: The mean follow-up time was 34 months (SD 12,9). The mean pretreatment IOP was 22.9 mm Hg (SD 2.1). At one month of follow-up, the mean IOP reduction was 5.3 mm Hg (SD 2.1) or 23.1 % and at 6 months 5.6 mm Hg (SD 2.6) or 24.5 %. At 12 months of follow-up, the mean IOP reduction was 5.6 mm Hg (SD 2.3) or 24.5 % and at 24 months 6.2 mm Hg (SD 2.5) or 27 %. At 36 months of follow-up, the mean IOP reduction was 6.4 mm Hg (SD 2.1) or 27.4 % and at the end of 48 months of follow-up, the mean IOP reduction was 5.9 mm Hg (SD 2.0) or 25.4 %. The succes rate after 12 months determined from the Kaplan-Meier survival analysis was 97 %, after 24 months 88 %, after 36 months 76 % and after 48 months 71 %. Conclusion: SLT is an effective procedure for the treatment of POAG.
