Koexistenz eines Barrett-Frühkarzinoms mit einem Granularzelltumor (Abrikosoff-Tumor) - Kasuistik und Review der Literatur

Zusammenfassung

Eine 40-jährige Patientin wurde uns zur Frage der endoskopischen Therapie eines hochdifferenzierten Barrettkarzinoms zugewiesen. Nach Ausschluss von Kontraindikationen erfolgte die Mukosaresektion mittels Inoue-Kappe. Überraschenderweise fand sich im Mukosektomiepräparat unterhalb des vollständig entfernten Karzinoms ein ebenfalls komplett entfernter Granularzelltumor innerhalb der Submukosa. Ob es sich bei diesem seltenen benignen submukösen Tumor des Ösophagus um eine Prädisposition für das Barrett-Karzinom in einem Short-Segment-Barrett handelt, kann nicht bewiesen, muss aber zumindest vermutet werden. Nach unseren Literaturrecherchen handelt es sich um den ersten publizierten Fall einer Koexistenz von Abrikosoff-Tumor und Barrett-Karzinom. Das Vorhandensein von Granularzelltumor und Plattenepithelkarzinom hingegen ist bereits öfter beobachtet worden.

Abstract

A 40y-old female patient was referred for endoscopic treatment of a highdifferentiated Barrett's carcinoma.
After ruling out contraindications the endoscopic mucosal resection was performed by using an Inoue-Cap.
Surprisingly the histological examination revealed a completely resected granular cell tumour within the submucosa below the also completely removed carcinoma.
Whether this rare benign submucosal esophageal tumour is a predisposition for a adenocarcinoma in a short-segment-Barrett's esophagus, cannot be proved but has to be at least suspected.
Reviewing the literature this is the first report of a coexisting Abrikosoff-tumour and a Barrett's carcinoma. The simultaneous occurrence of granular cell tumour and a squamous cell carcinoma was severally observed.
