Zusammenfassung

Gastroösophageale Refluxerkrankung und obstruktive Atemwegserkrankungen sind häufige Krankheitsbilder. Durch Literaturdaten und eigene Untersuchungen ist ein Zusammenhang durch bronchokonstriktive Reflexe oder/und einen ösophagobronchialen Reflux gesichert. Das optimale Management von Patienten mit COPD und GERD ist Gegenstand von Untersuchungen. Dieser Zusammenhang hat zur Folge, dass durch adäquate Behandlung der gastroösophagealen Refluxerkrankung eine Besserung chronisch-obstruktiver Lungenerkrankungen erreicht werden kann. Bei Patienten mit chronischen Atemwegserkrankungen unklarer Genese ist eine pH-Metrie indiziert, da in etwa 20 % der Patienten ein Zusammenhang zwischen Reflux- und chronischen Atemwegserkrankungen auch ohne Refluxsymptomatik nachgewiesen werden konnte.

Summary

Gastroesophageal reflux disease (GERD) and Chronic obstructive pulmonary disease (COPD) are extremely common. Recent data and our own results are indicating a pathophysiological correlation. Two mechanism are linking both diseases. First gastroesophageal reflux with or without microaspiration, and second the stimulation by refluxate of an esophagopulmonary/laryngeal reflex. Th optimal means of managing patients with COPD and GERD is yet to be determined. In nearly 20 percent of patients this correlation exists without typical symptoms of reflux. pH-Metry is therefore indicated in patients with obstructive lung disease of unknown origin. Adaquate treatment of GERD can improve the symptoms of chronic obstructive lung disease.
