Zusammenfassung

Im medizinischen Bereich wird der Wasserstrahl zur chirurgischen Präparation angewendet. Ziel dieser Untersuchung war es, die Anwendbarkeit des Wasserstrahls in der gastroenterologischen Endoskopie zu evaluieren. Das Wasserstrahlinstrumentarium wurde dazu dem gebräuchlichen Endoskopie-Equipment angepasst. Die Schneidversuche am Kadaverschwein lassen das Verfahren als ausreichend sicher erscheinen. Das Abtragen von Modellpolypen und das Zerkleinern von Speiseresten misslang immer dann, wenn das Zielobjekt dem Schneidstrahl ausweichen konnte. Die Umwandlung der kinetischen Energie des Strahles in Schneidkraft war so nicht möglich. Besonders erfolgversprechend erscheinen weitere Versuche an vitalen Strukturen zum Gebrauch des Wasserstrahls zur Deepithelialisierung.

Abstract

In industrial production the waterjet is known as a cutting technique. In dependance of pressure and diameter the waterjet may cut each material. In medicine the waterjet was successfull used in the preparation of liver, menigeoma, salivary gland, kidney and in reconstructive procedures in arthoscopy of the knee. The aim of this study was to evaluate the use of waterjet in gastrointestinal endoscopy. The waterjet was adapted to the common endoscopic aquipment. The use in dead pigs seems to be save. The cutting of potatoes and meet in the stomac was not possible. Cutting of artificial polyps was impossible when the object escaped the waterjet. In this case the transformation of kinetic energy in cutting power was not given. Special interest of waterjet cutting should be given to the study of deepithelisation in vital tissue.
