Psychopharmakotherapie der posttraumatischen Belastungsstörung

Zusammenfassung

Welchen Stellenwert hat die Psychopharmakotherapie für die Behandlung der posttraumatischen Belastungsstörung (PTBS)? Wie lassen sich Psychopharmaka - bei entsprechender Indikation - optimal in die psychotherapeutische Behandlung einfügen? Mit diesen Fragen befasst sich die vorliegende Arbeit. Während die Psychopharmakotherapie der PTBS bislang eher symptomorientiert und eklektisch verfährt, plädieren die Autoren für ein Vorgehen, das ätiopathogenetisch ausgerichtet ist, den Verlaufsprozess der Störung berücksichtigt und auf ihre zentrale Bio- und Psychodynamik abgestimmt ist. Es wird der Grundsatz vertreten, dass die Psychopharmakotherapie als Ergänzung zur Psychotherapie gesehen werden sollte und schweren Fällen vorbehalten ist. Im Studienvergleich gelten selektive Serotonin-Rückaufnahmehemmer als Pharmakon der Wahl.

Psychopharmacotherapy of Posttraumatic Stress Disorder

What is the position and value of psychopharmacology in the treatment of posttraumatic stress disorder (PTSD)? How are pharmaceutics, under proper indications, optimally included in psychotherapeutic treatment? These are the topical questions which the present article addresses. Until now, psychopharmaceutical therapy of PTSD has been mainly symptomatic and eclectic. Here we argue for an approach that: a) is aetiopathogenically oriented, b) pays attention to the course of the disorder, and c) is attuned to its central biopsychodynamics. The basic principle advocated here is that psychopharmacotherapy: a) should be seen not as replacement of but rather as complement to psychotherapy, and b) is reserved for severe cases.

In the guidelines to treatment of PTSD, medication of selective serotonin reuptake inhibitors have proved to be very valuable.
