Zusammenfassung

Die proximale Verriegelung von Humerusmarknägeln stellt eine mögliche Verletzung des N. axillaris dar. Wenn auch klinische Erfahrungen dies nicht zu bestätigen scheinen, so soll diese Frage anatomisch-morphologisch geklärt werden.
Bei einer Untersuchung an 8 Schulter-Oberarm-Präparaten zeigt sich, dass die latero-medial eingebrachten Bolzenköpfe am weitesten vom medio-dorsal gelegenen Stamm des N. axillaris entfernt sind. Dorso-ventral eingebrachte Bolzen stehen allerdings in unmittelbarer Beziehung zum Axillarisstamm. Diese Relationen finden sich auch bezüglich des Bolzenkanales im M. deltoideus zu den nächstgelegenen Axillarisästen. Die Exploration des N. axillaris und seiner Verzweigungen ergab in keinem Fall eine direkte Läsion. Der dorso-ventrale Bolzen sollte somit nur in Ausnahmefällen Verwendung finden. Wichtig ist, beim Zugang lediglich die Haut zu inzidieren, den darunter liegenden Muskel vorsichtig stumpf bis auf den Knochen zu spreizen und beim Bohren zu retrahieren.

Injury of the axillary nerve due to proximal interlocking of intramedullary humeral nails

Summary

Proximal interlocking of the humeral shaft could lead to injuries to the axillary nerve, although clinical experience seems not to support this theory. This study will prove the risk under anatomical-morphological aspects.
Analysing eight cadaveric shoulder-arm regions with inserted interlocking humeral nails shows, that the latero-medially inserted bolt heads are being placed most distant to the medio-dorsally positioned stem of the axillary nerve. Dorso-ventrally inserted bolts are presenting a very tight relation to the nerve stem. Exploring the axillary nerve and its branches showed in no case a direct nerve lesion. The dorso-ventral bolt should only be used exceptionally. Only a sharp cutaneous incision should be performed and the muscle must be prepared only by careful spreading until touching bone, with tissue retraction during drilling.
