Klinische Bedeutung der Nagelung in der Traumatologie[]

Zusammenfassung

Obwohl Küntscher die Nagelung von Knochenbrüchen bereits Anfang der vierziger Jahre in den klinischen Alltag einführte, wurde diese Art der Osteosynthese erst in den siebziger Jahren besser akzeptiert. Die Anlaufphase war gekennzeichnet von technischen Problemen, einer ziemlich hohen Rate an Komplikationen und einem sehr engen Spektrum an Indikationen.
Die Einführung und der Erfolg des Gammanagels zur Versorgung von Hüftfrakturen im hohen Lebensalter hat wesentlich zu der allgemeinen Akzeptanz der intramedullären Nagelung beigetragen. Das Spektrum der Indikationen entwickelte sich von der Behandlung einfacher Schaftfrakturen zu komplexeren Schaftfrakturen, metaphysären Verletzungen und Läsionen mit erheblichem Weichteilschaden.
In der näheren Zukunft wird die Forschung sich neuen Materialien, neuen Indikationsgebieten und Nagelung mit Hilfe der Navigation widmen.

Clinical significance of intramedullary nailing in traumatology

Summary

Although nailing was introduced by Küntscher in clinical practice in the early fortties, it took until the seventies before this type of osteosynthesis found more acceptance. The early period was characterized by technical problems, a rather high ratio of complications, and a very small spectrum of indications.
The introduction and success of Gamma-nailing for hip fractures in elderly patients helped significantly to the general acceptance of nailing. From the treatment of simple shaft fractures, the spectrum of indications grew to complex shaft fractures, metaphyseal fractures, and lesions with important soft tissue damage. In the near future research work will focus on new materials, other indications, and nailing with help of navigation tools.

Résumé

Bien que l'enclouage des fractures ait été introduit par Küntscher dès les années quarante, ce mode d'ostéosynthèse n'a été bien accepté que dans les années soixante dix. La phase de démarrage a été marquée par des problèmes techniques, un taux relativement élévé de complications et des indications très limitées.
L'introduction et le succès du clou gamma dans le traitement des fractures de hanche chez le sujet âgé a fortement contribué à l'acceptation généralisée de l'enclouage centro-médullaire. Le spectre des indications s'est étendu des fractures diaphysaires simples aux fractures complexes diaphysaires métaphysaires et aux lésions avec importants dégâts des parties molles.
Dans un avenir proche la recherche s'orientera vers de nouveau matériaux, de nouveaux domaines d'indication et vers l'enclouage à l'aide de la navigation.

Resumen

Aunque el enclavijado fue introducido por Küntscher en la práctica clínica al inicio de los cuarenta, fue necesario llegar hasta los setenta para que este tipo de osteosíntesis encontrara mayor aceptación. El período inicial se caracterizó por problemas técnicos, una incidencia relativamente alta de complicaciones, y un espectro muy reducido de indicaciones.
La introducción y éxito del clavo Gamma para las fracturas de cadera en los pacientes añososa ayadoe significativamente a la aceptación general del enclavijado. Del tratamiento de fracturas diafisarias simples, el espectro de indicaciones creció a fracturas diafisarias complejas, fracturas metafisarias, y lesiones con importante daño de las partes blandas. En el futuro próximo la investigación se focalizará en nuevos materiales, otras indicaciones, y el enclavijado con la ayuda de navegación.
