Zusammenfassung

Der Ersatz des vorderen Kreuzbandes mit Patellarsehne und Fixation mit Cross pin („Rigid fix”) hat gegenüber anderen Verfahren einige Vorteile: 1. Hohe Steife des Transplantates bei geringem Transplantatdurchmesser. Dadurch wird die Gefahr eines Transplantatimpingement am hinteren Kreuzband und an der lateralen Sulkuswand verringert. 2. Tunnelweitung femoral wird vermieden durch gelenknahe (anatomische) Fixation des Knochenblocks und die Umlenkung der Sehne am Knochenblock. Die vollständige knöcherne Auffüllung des tibialen Bohrkanals im Transplantatverlauf durch Spongiosaplastik ist eine wichtige Maßnahme, um Tunnelweitung im Tibiakopf zu vermeiden. 3. Wenige, gut standardisierte Fixationsmethoden. Hohe Stabilität der Primärfixation mit allen Fixationstechniken. Dadurch niedrige Fehlerquote. 4. Durch Fixation mittels „Rigid fix” Vermeidung von Hohlräumen, wie sie nach Resorption von Interferenzschrauben entstehen können. 5. In zahlreichen Studien nachgewiesene gute Langzeitstabilität. Die Morbidität durch die Entnahme des Transplantates läßt sich durch verbesserte Operationstechnik weitgehend vermeiden: 1. Doppelinzision, quer über der Patella und schräg über dem Tibiakopf mit subkutaner Sehnenentnahme führt zu guter Narbenbildung und schont die R. infrapatellares n. sapheni. 2. Spongiosaauffüllung der Tuberositas tibiae und des unteren Patellapols vermeidet oder verringert die Beschwerden beim Knien. 3. Die hohe Primärstabilität erlaubt frühzeitig intensives Quadrizepstraining. Dadurch reduziert sich das Auftreten einer Chondropathia patellae.

The replacement of the anterior cruciate ligament with patella tendon

Summary

The replacement of the anterior cruciate ligament with patella tendon and “Cross pin” fixation comprises several advantages compared to others techniques: 1. High stiffness and low diameter of the transplant. The risk of transplant impingement to the posterior cruciate ligament an the lateral wall of the intercondylar sulcus is therefore reduced. 2. The femoral tunnel enlargement can be avoided with anatomic fixation of the transplant close to the joint line and dealignment of the tendon at the bone graft. It is important to transplant autologous spongiosa into the tibial tunnel to avoid tunnel enlargement. 3. Fixation with the rigid fix system avoids development of bone cysts that may occur after use of interference screws. 4. Many studies have shown good long term results concerning the stability of the anterior cruciate ligament fixation. The morbidity after graft harvesting can be reduced with advanced operation technique: 1. Double incision (patella and tibial tuberosity) with subcutaneous graft harvesting results in good scars and avoids damage of the infrapatellar nerves. 2. Spongiosa transplantation into the tibial and patellar graft side avoids or decreases pain at squatting. 3. High primary stability of the ligament fixation allows early quadriceps muscle rehabilitation, therefore the incidence of femoropatellar pain can be reduced.

Résumé

Le remplacement du ligament croisé antérieur par le tendon rotulien avec fixation par Cross-pin (Rigid fix) présente quelques avantages par rapport à d'autres méthodes: 1. Importante rigidité du transplant sur faible diamètre. De ce fait le danger de conflit avec le ligament croisé postérieur et au niveau du mur latéral de l'échancrure intercondylienne est réduit. 2. L'élargissement du canal fémoral est évité grâce à la fixation anatomique du bloc osseux. Il est important de combler le tunnel tibial avec de l'os spongieux afin d'éviter son élargissement. 3. Peu de méthodes de fixation bien standardisées. Bonne stabilité des fixations primaires avec toutes les techniques. Peu d'échec. 4. Grâce à la méthode «Rigid Fix», le développement des cavités de résorptions telles qu'elles existent avec des vis d'interférence est évité. 5. De nombreuses études confirment les bons résultats à long terme.
La morbidité au niveau de la prise du transplant peut-être largement réduite grâce à une technique opératoire améliorée: 1. La double incision sur la rotule et la tubérosité tibiale avec prise de greffon sous-cutanée assure une bonne cicatrisation et évite les lésions des rameaux infrapatellaires du nerf saphène. 2. Le comblement avec du spongieux de la tubérosité tibiale et des sites de prélèvement patellaire évite ou diminue la gêne lors de la génuflexion. 3. L'excellente stabilité primaire de la fixation permet une rééducation précoce et intensive du quadriceps. De la sorte le développement d'une chondropathie de la rotule est réduit.

Resumen

El reemplazo del ligamento cruzado anterior con tendón patelar y la fijación “cross pin” comprende varias ventajas en relación a otras técnicas: 1. Alta resistencia y pequeño diámetro del transplante. El riesgo de roce del transplante sobre el ligamento cruzado posterior y la pared lateral del surco intercondíleo es así reducida. 2. El ensanchamiento del túnel femoral puede evitarse con la fijación anatómica del transplante cercana a la interlínea articular y la alineación del tendón en el injerto óseo. Es importante transplantar esponjosa autóloga en el túnel tibial para evitar el ensanchamiento del túnel. 3. La fijación con sistemas rígidos evita el desarrollo de quistes óseos que puede ocurrir en el uso de tornillos de interferencia. 4. Muchos estudios han mostrado buenos resultados a largo plazo en cuanto a la estabilidad de la fijación del ligamento cruzado anterior.
La morbilidad luego de la toma del injerto puede ser reducida con técnica operatoria de avanzada: 1. Incisión doble (rótula y tuberosidad tibial) con toma subcutánea del injerto resulta en cicatrices correctas y evita el daño de los nervios infrarotulianos. 2. El transplante de esponjosa en el componente tibial y rotuliano evita o disminuye el dolor al realizar squat. 3. La alta estabilidad primaria de la fijación ligamentaria permite rehabilitación cuadricipital precoz, con reducción de la incidencia de dolor patelo-femoral.
