Duodenal Lipoma-The Real Troublemaker
Abstract Text This is the case of a 56 year old Caucasian woman, who presented in the emergency room for melena. We proceeded with an endoscopy, finding a polypoid-like lesion in the duodenum, with ulcerated mucosa, that most likely was the source of the hemorrhage. We started by placing 2 Lockado clips at the base of the polyp followed by using the diathermy, in order to cut the lesion. For hemostasis, another Lockado clip was placed. The patient returned to the emergency room with abdominal intense pain 5 days later. A new endoscopy was performed, finding a late perforation opening of approximately 15 mm, that was closed by using multiple Lockado clips. The abdominal pain persisted, so we performed another endoscopy. Consequently, a final decision was reached, resulting in the application of a Padlock clip precisely on the perforation opening, entirely closing the parietal mucosal defect [1] [2] [3] [4].
