Knöcherne Verletzungen der Rippen sind häufige Verletzungen des Brustkorbs und werden meist, wie etwa bei Verkehrsunfällen, durch eine direkte und stumpfe Gewalteinwirkung hervorgerufen. Demgegenüber sind Sternumfrakturen ohne begleitende Rippenfrakturen, wie etwa bei polytraumatisierten Patienten, mit ca. 4% selten, wobei sie in Kombination mit Rippenfrakturen in bis zu 21% auftreten können.

Kernaussagen
  • Die Behandlung sowohl der Rippen- als auch Sternumfrakturen richtet sich meist nach dem Auftreten und Ausmaß der thorakalen (z. B. Pneumothorax, Lungen-, Gefäßverletzung) und abdominellen (Milz-, Nieren-, Leberverletzung) Begleitverletzungen, welche zwingend ausgeschlossen werden müssen.

  • Bei Sternumfrakturen sollte darüber hinaus insbesondere an Begleitverletzungen von Brustwirbelsäule und Herz gedacht werden.

  • Aufgrund der gegebenenfalls schwerwiegenden Verletzungen nach einem Thoraxtrauma sollte die Diagnostik entsprechend der ATLS®-Richtlinien erfolgen und neben klinischer und radiologischer Diagnostik auch Ultraschall und EKG miteinschließen.

  • Rippenserienfrakturen mit respiratorischer Einschränkung sind eine schwerwiegende Verletzung und müssen stationär, ggf. unter intensivmedizinischen Bedingungen, überwacht werden.

  • Da Sternumfrakturen in der Regel mit weiteren Begleitverletzungen einhergehen, ist eine 24-stündige stationäre Überwachung stets in Erwägung zu ziehen.

  • Die Therapie der Sternum- und Rippenfrakturen ist überwiegend konservativ.

  • Eine suffiziente Analgesie einhergehend mit einer Atemtherapie ist wesentlicher Bestandteil der konservativen Therapie von Rippen- und Sternumfrakturen.

  • Die operative Stabilisierung von Rippenserienfrakturen mit respiratorischen Einschränkungen nimmt einen zunehmenden Stellenwert in derer Behandlung ein.
