Kernaussagen

Der regelrechte Pubertätsbeginn hängt von ethnischen, genetischen und sozioökonomischen Faktoren ab. Als vorzeitiger Pubertätsbeginn wird das Auftreten von Thelarche und Pubarche vor dem 8. Geburtstag und/oder Menarche vor dem 9. Geburtstag bezeichnet. Grundsätzlich werden 2 Pathomechanismen unterschieden:

1. Die echte, zentrale oder GnRH-abhängige Pubertas praecox (ZPP) ist Folge einer vorzeitigen pulsatilen Sekretion von GnRH mit nachfolgender Aktivierung der Gonadotropine LH und FSH.

2. Die periphere, Pseudo- oder GnRH-unabhängige Pubertas praecox (PPP) wird durch erhöhte Sexualsteroidproduktion unterschiedlicher Ursache verursacht. GnRH und die Gonadotropine spielen dabei keine Rolle.

Therapeutische Interventionen bei der verfrühten Pubertät haben das Ziel, die vorzeitige körperliche Entwicklung und akzelerierte Knochenreifung aufzuhalten. Dies geschieht bei der GnRH-abhängigen Form mit GnRH-Agonisten oder bei der GnRH-unabhängigen Pubertas praecox mit einer für die zugrunde liegende Ursache spezifischen Therapie.
