Percutaneous Closure of a Ruptured Sinus of Valsalva's Aneurysm in a Syndromatic Child with a New-Type VSD Device

Objectives: Sinus of Valsalva's aneurysms can present clinically dramatic in case of rupture. Acute shunts can develop toward the right atrium or ventricle or toward other cardiac structures. Closure can be achieved either surgically or percutaneously.

Methods: A 14-year-old girl with trisomy 13 presented with acute deterioration. Pulmonary hypertension was suspected, and a heart murmur was detected. Echocardiography showed a ruptured sinus of Valsalva's aneurysm, which open end into the right atrium close to the tricuspid valve. As surgery seemed high risk, closure was performed percutaneously using a new double-disk device marketed for VSD closure which was delivered through a 5-F guiding catheter from the aortic side.

Result: The child could be extubated 2 hours after the procedure and recovered quickly.

Conclusion: Interventional closure of ruptured sinus of Valsalva's aneurysms is an attractive option and can be performed safely. To our recent knowledge, this is the first report of interventional closure of such defect in a trisomy 13 patients, and the first time, such closure was performed this specific device.
