Zusammenfassung

Die Weiterentwicklung der OCT-Technik ermöglicht durch eine Verrechnung (SSADA = Split-Spectrum-Amplitude-Decorrelation-Algorithmus) von 2 rasch aufeinander aufgenommenen Scans – es wird somit nur dargestellt, wo sich etwas verändert hat und fließt – in den neuesten Geräten eine Darstellung von retinalen und chorioidalen Gefäßen. Diese neue Technik der „OCT-Angiografie“ ermöglicht nicht invasiv innerhalb von Sekunden eine 3-dimensionale Rekonstruktion der gesamten vaskulären Strukturen der Netzhaut und Aderhaut. Noch ist diese Technik durch Bewegungsartefakte, Überlagerungen und zu entwickelnden 3-dimensionalen Abbildungsmodalitäten in ihrem letztendlichen Potenzial begrenzt, aber erste Erfahrungen und Korrelationen mit Befunden in dem bisherigen Standarddiagnoseverfahren der Fluoreszeinangiografie zeigen, dass besonders bei flächigen Veränderungen wie der diabetischen Retinopathie oder retinalen Venenverschlüssen eine sehr gute Korrelation gegeben ist. Bei Patienten mit makulären Teleangiektasien (Typ 2) können darüber hinaus sogar die bisher vermuteten Gefäßveränderungen im tiefen Kapillarnetz der Netzhaut wesentlich besser und stadienabhängig progredient dargestellt werden. Demgegenüber müssen für eher 3-dimensionale Gefäßveränderungen, wie sie z. B. bei der exsudativen AMD vorliegen, noch diagnostische Strategien evaluiert und festgelegt werden. Doch erste Befunde deuten darauf hin, dass auch in der OCT-Angiografie ähnliche Differenzierungen wie in der Fluoreszeinangiografie möglich sind, darüber hinaus aber zusätzliche Informationen über die assoziierten Veränderungen in der Aderhaut und äußeren Netzhaut gewonnen werden können, die zu einem besseren Verständnis dieser Pathologien führen können.

Abstract

Recent developements in OCT technology using high speed acquisition and calculation of consecutive scans (SSADA = split spectrum amplitude decorrelation algorithm) have resulted in the possibility to demonstrate retinal and choroidal vessels in the macula. This new technology of “OCT angiography” thus allows the non-invasive and rapid (within seconds) reconstruction of the three-dimensional structure of the retinal and choroidal vascularisation. There are still limitations caused by movement artefacts, superposition of superficial retinal vessels at the RPE level or insufficient three-dimensional imaging, but the first experience with this new method and especially the correlations with the current standard diagnostic procedure fluorescein angiography shows that especially for vascular changes which are predominantly in one retinal layer (e.g., the inner retina) like in diabetic retinopathy or retinal vein occlusions, a very good correlation can be seen. Also in MacTel type 2 patients the proposed vascular changes in the deeper capillary network of the retina can be visualised very well with OCT angiography. In contrast, more three-dimensional vascular changes like the neovascular complex in exsudative AMD need a more sophisticated diagnostic analysis strategy, which has still to be developed. However, the first experience also demonstrates that fluorescein angiographic differentiation can also be seen in OCT angiography. In addition, the new technology gives additional information about the choroidal and outer retinal changes in these pathologies, which may result in a better understanding of the underlying pathologies.
