Zusammenfassung
Die Rezidivierende Polychondritis (RP) ist eine seltene entzündlich rheumatische System-erkrankung mit Befall von Knorpel- und Bindegewebe mit je nach Lokalisation entsprechender Organbeteiligung. Pathognomonisch ist eine Chondritis an Ohr- und Nasenknorpel. Eine gehäufte Assoziation mit anderen Autoimmunerkrankungen sowie immunhistologische/ immunologische Befunde sprechen für eine autoimmune Genese. In der Regel ist der Verlauf chronisch rezidivierend und mündet bei insuffizienter Therapie oder refraktären Verläufen in Destruktion der befallenen Areale. Die Therapie besteht aus Steroiden und in Abhängigkeit von Lokalisation/Ausmaß zusätzlich aus mittel- bis hochpotenten Immunsuppressiva. In den letzten Jahren hat sich die Prognose durch frühzei tigere Diagnosestellung und Einsatz von Biologika bei refraktären Verläufen deutlich gebessert.
Summary
Relapsing polychondritis is a rare inflammatory disorder which involves cartilaginous structures of different organs. Auricular and nasal chondritis are pathognomonic for this disease. Its relapsing-remitting course often leads to destruction of the involved tissue. Therapy usually consists of steroids and dependent on severity various steroid sparing agents such as DMARDs and biologics. In the last years the prognosis improved due to an earlier diagnosis in combination with an increasing armamentarium of highly potent immunomodulating medicaments, especially biologics.
