Zusammenfassung
Mit der Einführung des G-DRG-Systems hat die Dokumentation und Kodierung erheblich
an Bedeutung gewonnen. Durch die Etablierung spezifischer ICD- und OPS-Kodes konnte
eine deutlich sachgerechtere Finanzierung rheumatologischer Behandlungsinhalte erzielt
werden. Trotz Bemühungen des Verbandes Rheumatologischer Akutkliniken über einen jährlich
aktualisierten Kodierleitfaden die Kodierung rheumatologischer Fälle zu vereinheitlichen,
wird lokal zwischen Krankenhäusern und Kostenträgern zunehmend über die korrekte Kodierung
und Dokumentation gestritten. Es bedarf erheblicher Anstrengungen der systemverantwortlichen
Institutionen, wenn die für die Kodierung, Dokumentation und Abrechnung eingesetzten
Ressourcen wieder in die Versorgung der Patienten zurück überführt werden sollen.
Summary
The importance of documentation and coding has risen considerably after the introduction
of the G-DRG-system. By establishing specific ICD-codes and OPS-codes for diagnoses
and procedures the rheumatologic inpatient treatment experienced a much more appropriate
representation in the G-DRG system. The organization of rheumatologic hospitals (VRA)
publishes a coding manual, which is updated every year. Despite these efforts to harmonize
the coding of rheumatologic inpatient treatment, the dispute between hospitals and
insurance companies about the correct coding and documentation increases further.
A concerted commitment of the responsible stakeholders is required to reallocate resources
from coding, documentation and billing back to the treatment of patients.
