Tolvaptan – Hyponatriämie bei SIADH gezielt behandeln

Tolvaptan ist der erste und derzeit einzige in Europa verfügbare orale selektive V₂-Vasopressin-Rezeptor-Antagonist, der zur gezielten Behandlung von Erwachsenen mit Hyponatriämie als Folge eines SIADH (Syndrom der inadäquaten Sekretion des antidiuretischen Hormons) zugelassen ist. Die Hyponatriämie ist mit einer Prävalenz von 15–30 % bei hospitalisierten Patienten die häufigste Elektrolytstörung [ 1 ], [ 2 ]. Unerkannt und unbehandelt ist vor allem die schwere Hyponatriämie mit einer erhöhten Morbidität, verlängerten Krankenhausaufenthalten sowie erhöhten Kosten assoziiert [ 3 ], [ 4 ], [ 5 ]. Aber auch milde, vermeintlich asymptomatische Hyponatriämien können zu erheblichen neurokognitiven und motorischen Störungen führen [ 6 ].
