Zusammenfassung

Die Tuberkulose des Ösophagus ist selten. Wir berichten über einen Patienten mit mediastinaler und ösophagealer Tuberkulose in Kombination mit einer Meningitis tuberkulosa. Die initialen Symptome des Patienten waren Fieber, Gewichtsverlust, Kopfschmerzen und Doppelbilder. In der CT-Untersuchung des Thorax fand sich eine Raumforderung im Mediastinum. Die Diagnose konnte durch eine endosonografisch gezielte Punktion der mediastinalen Raumforderung gestellt werden. Trotz antituberkulöser Therapie verschlechterte sich der Zustand des Patienten, er entwickelte ein Querschnittssyndrom und sitzt heute im Rollstuhl.

Abstract

Esophageal tuberculosis is rare. Here we report a patient with mediastinal and esophageal tuberculosis in combination with meningitis tuberculosa. The patient presented with fever, weightloss, headache and double vision. On computer tomography a mediastinal mass was detected. The suspected diagnosis was confirmed by endosonographic-guided fine needle aspiration of the mediastinal mass. Despite anti-tuberculosis drug therapy tuberculosis meningitis deteriorated and the patient needs a wheelchair on account of his paraplegia.
