Zusammenfassung
Menschen mit Amyotropher Lateralsklerose (ALS) benötigen umfassende Hilfen für Mobilität,
Kommunikation, Ernährung, Beatmung und andere Versorgungselemente. Sie erfahren eine
Unterstützung von verschiedenen Akteuren. Die Abstimmung der unterschiedlichen Partner
ist komplex und mit hohen Barrieren verbunden. AmbulanzPartner (AP) bietet ALS-Betroffenen
und allen Akteuren ein Netzwerk für eine koordinierte Hilfs- und Heilmittelversorgung,
die ein Case Management und das Internetportal AmbulanzPartner.de in einem hybriden Produktkonzept vereint. AP reduziert bestehende Schnittstellen
in der ALS-Versorgung. Das Internetportal AmbulanzPartner.de dient als „elektronische Versorgungsakte“ und „Managementplattform“, die Patienten,
Angehörige, Ärzte und Versorgungspartner verbindet und alle Versorgungsprozesse erkennbar
macht. AP dient dazu, die Versorgung mit Hilfs- und Heilmitteln zu erleichtern und
Fehlversorgung zu vermeiden. Das Managementportal ist seit April 2011 im Produktivbetrieb.
Bis zum März 2013 wurden 2 363 Patienten und 15 867 Hilfs- und Heilmittelversorgungen
koordiniert. Die erhobenen Daten dienen der Versorgungsforschung und bieten Einblick
in die Versorgungsprozesse bei unterschiedlichen klinischen und sozialmedizinischen
Konstellationen sowie eine vergleichende Betrachtung von Hilfsmittelgruppen und Kostenträgern.
Die Nutzung des Internetportals AmbulanzPartner.de ist mit einem Erlösmodell verknüpft und folgt dabei dem „Shared Value“-Prinzip: Patienten,
Angehörige, Ärzte und sozialmedizinische Partner nutzen das Portal kostenlos, während
die unternehmerischen Akteure (z. B. Hilfsmittelversorger) in ein Lizenzmodell eingebunden
sind. AP beinhaltet einen generischen Ansatz und eine Skalierbarkeit, sodass – ausgehend
von der ALS – eine Transferfähigkeit für andere komplexe neurologische Erkrankungen
besteht.
Abstract
Patients suffering from amyotrophic lateral sclerosis (ALS) require comprehensive
aids for mobility, communication, nutrition, ventilation and other care elements.
They receive support from a variety of stakeholders. Coordination of the various partners
is a complex matter and subject to high barriers. AmbulanzPartner (AP) provides persons
afflicted with ALS and all players involved with a network for the coordinated provision
of assistive devices and therapies combining case management and the internet portal
AmbulanzPartner.de in a hybrid product concept. AP reduces the existing interfaces in ALS care. The
internet portal AmbulanzPartner.de serves as an “electronic health record” and “management platform” liaising between
patients, next of kin, doctors and providers while making all care processes discernible.
AP is designed to facilitate the provision of assistive devices and therapies and
to avoid inappropriate health care. The management portal has been in operation since
April 2011. Up to March 2 013, 2 363 patients and 15 867 assistive devices and therapies
have been coordinated. The captured data are designated for health-care research and
offer an insight into care processes for different clinical and sociomedical constellations
as well as a comparative consideration of arrays of assistive devices and insurance
companies. Use of the internet portal AmbulanzPartner.de is based on a “shared value” principle revenue model: patients, next of kin, doctors
and sociomedical partners, may use of the portal free of charge whereas the entrepreneurial
stakeholders (e. g., providers of assistive devices) are integrated into a licensing
model. AP embraces a generic approach and scalability so that – based on ALS – transferability
to other complex neurological conditions is given.
