Akuter Myokardinfarkt – Was empfehlen die neuen ESC-Leitlinien?

Die „2012 ESC-guidelines for the management of acute myocardial infarction in patients presenting with ST-segment elevation“ (STEMI) unterscheiden sich zu denen von 2008 hinsichtlich der Benennung von Qualitätszielen bei der interventionellen Behandlung des akuten Myokardinfarkts (STEMI), der Bevorzugung von Drug-Eluting-Stents, der Empfehlung des radialen Zuganges und der Herabstufung der Wertigkeit der Beta-Rezeptorenblocker nach Myokardinfarkt. Thrombozyten-Aggregationshemmung und Antikoagulation werden ausführlich dargestellt und diskutiert. Zu begrüßen ist die klare Darstellung des qualitativen Ablaufes einer Primär-PTCA und die Benennung von klaren Zeitlimits, die die Kliniken zu beachten haben. Weniger überzeugend ist die Bevorzugung der Drug-Eluting-Stents und des radialen Zugangs. Bedenklich ist die verfrühte Festlegung auf neue Antikoagulantien.

The „2012 ESC-guidelines for the management of acute myocardial infarction in patients presenting with ST-segment elevation“ (STEMI) differ in respect to the 2008 version by clarifying the quality targets of interventional therapy of acute myocardial infarction., the recommendation of drug eluting stents and the radial approach and the degrading of the value of betareceptor blockers. Antiplatelet therapy and anticoagulation are extensively described and discussed. The clear description how to perform PCI in acute myocardial infarction and the description of clear time limits which must be observed are appreciated. Less convincing is the recommendation of drug eluting stents and the radial approach. Worrying are the premature recommendations of new anticoagulants.
