Zusammenfassung
Bei 680 langjährig beobachteten Patienten einer internistisch-psychosomatischen Praxis
wurde der Krankheitsverlauf bei Kassen- und Privatpatienten verglichen. Dabei wurden
charakteristische Unterschiede festgestellt. Während der Anteil der negativen Verläufe
bei beiden Patientengruppen mit 12,6% gleich hoch ist, liegen die positiven Verläufe
mit 8,9% bei den Privatpatienten um fast das Sechsfache höher als bei den Kassenpatienten,
bei denen sie nur 1,5% betragen. Daher muß angenommen werden, daß der Versicherungsmodus
den Interaktionsprozeß zwischen Arzt und Patient und damit auch den Krankheitsverlauf
entscheidend mitbeeinflußt.
Summary
The course of illness of 680 patients treated over several years in a general medical
and psychosomatic practice was analysed. Characteristic differences were found between
privately and state-insured patients. While the proportion of »negative« cases (worse,
markedly worse, dead) was the same in both groups at 12.6%, »positive« results (much
improved, improved) was much higher among private (8.9%) than state-insured patients
(1.5%). It is concluded that the insurance status decisively influences the relationship
between doctor and patient and with it the course of illness.
Resumen
Estudio comparativo sobre el curso de la enfermedad en pacientes privados y con seguro
social
En 680 pacientes observados a lo largo de varios años de un consultorio de medicina
interna-psicosomática se comparó el curso de la enfermedad en los pacientes con seguro
de enfermedad y privados. Se encontraron diferencias características. En tanto que
el porcentaje de evoluciones negativas era igual en ambos grupos con un 12,6%, eran
los cursos positivos en los pacientes privados seis veces más altos, con un 8,9%,
que en los pacientes con seguro de enfermedad, en los que sólo alcanzó un 1,5%. Así
pues tiene que admitirse que el tipo de seguro influye también decisivamente sobre
el proceso interactivo entre el médico y el paciente y con ello también sobre el curso
de la enfermedad.
