Beitrag zur Echinokokkose des Herzens an Hand von sieben eigenen Beobachtungen

Zusammenfassung

Die kardiale Lokalisation des Echinococcus hydatidosus ist mit ca. 1% aller Echinokokkosen äußerst selten. In der Differentialdiagnostik ist sie jedoch gegenüber den intrakardialen Tumoren und unklaren „Koronaropathien” jugendlicher Individuen von großer Bedeutung. Unser Krankengut umfaßt 7 Patienten mit insgesamt 14 Zysten, von welchen 8 im linken, 4 im rechten Herzen und 2 im Kammerseptum lokalisiert waren. 5 Patienten zeigten das klinische Bild des „Komplikationsstadiums” mit Ruptur der Zyste in die Umgebung, wobei 3 Durchbrüchen ins Perikard 2 in die Herzhöhle gegenüberstanden. Zur Operation empfehlen wir die mediane Sternotomie und die routinemäßige Anwendung der HLM. Von unseren Patienten wurden 4 ohne und 3 unter Zuhilfenahme der EKZ operiert. 2 Todesfällen stehen 5 Heilungen gegenüber.

Summary

The localisation of echinococcus hydatidosus in the heart will be found in about 1% of all echinococcosis. It is however important to be distinguished in differential diagnosis from the intracardiac tumors or an obscure “coronaropathy” of young patients.

In seven cases fourteen cysts had been observed. Eight were localised in the left, four in the right heart, and another two in the ventricular septum. Five patients showed the clinical picture of the “stadium of complications” with three ruptures in the pericardium and two in the cardiac cavity. For operation we recommend median sternotomy and extracorporeal circulation (ECC). Four of our patients had been operated without and three with ECC.

Two of the patients died, five were cured.
