Zur Korrektur des aortopulmonalen Septumdefektes

Zusammenfassung

Anhand der Beobachtung eines aortopulmonalen Septumdefektes bei einem 2 Jahre, 4 Monate alten Kind wird über die Diagnostik und operative Korrektur berichtet. Die Korrektur erfolgte unter Hypothermieanwendung in einer kurzfristigen Einflußsperre mit Hilfe einer mechanischen Naht. Der technisch einfache und wenig aufwendige Eingriff erwies sich als risikoarm und führte zu einer völligen Normalisierung der hämodynamischen Verhältnisse.

Diese bisher nach dem Schrifttum nicht praktizierte Methode wird als geeignete Korrekturoperation im Kleinkindesalter empfohlen.

Summary

Diagnosis and correction are discussed on the basis of an aorto-pulmonary septal defect in a 2⁴/₁₂ year old child. Correction was carried out under hypothermia with brief occlusion of the blood supply by mechanical suture. The operation which was technically simple proved to be of low risk and led to complete normalisation of haemodynamic conditions.

This method which has not been used in the literature up to the present is recommended as a suitable corrective operation in young children.

L'observation du cas d'un vice de la cloison ao to-pulmonaire chez un enfant de deux ans et quatre mois incite l'auteur à étudier le diagnostic de cette affection et l'opération correctrice. L'intervention a été entreprise sous hypothermie avec une brève interruption de l'afflux sanguin à l'aide d'une suture mécanique. L'intervention à la technique simple et peu astreignante présente peu de isques etpermet; une normalisation totale de la situation hémodynamique.

Cette méthode encore inconnue dans la littérature est recommandée comme le traitement de choix chez l'enfant en bas âge.
