Zusammenfassung
Zum Ersatz von Trachea und großen Bronchien haben sich im Tierexperiment weder homoioplastische Transplantate noch Rohre aus festem Material, Stahl- oder Tantalnetze, ob sie mit oder ohne autologe Gewebehüllen eingesetzt wurden, bewährt. Die besten Ergebnisse lieferten bis heute hochporöse Kunststoffe, wie Marlexnetze und Teflonprothesen, welche durch eine eingeschweißte Spirale aus Niederdruckpolyäthylen stabilisiert sind. Die letzten Prothesen werden intraperitoneal für drei bis vier Wochen implantiert. Trotz zahlreicher Experimente finden sich in 50 % noch Früh- und Spätkomplikationen, etwa in 25 % Infektionen und Narbenschrumpfungen mit Stenosen.
Für den Gebrauch in der Humanmedizin erscheint der alloplastische oder auto-alloplastische Trachealersatz noch nicht geeignet, auch wenn vereinzelt über positive Resultate berichtet wird.
Summary
Neither homoplastic grafts nor tubes of solid material, steel or tantalium mesh whether implanted with or without a covering of autologous tissue have proved to be of value in the replacement of the trachea and large bronchi. Up to the present, the best results have been obtained with highly porous plastics such as Marlex mesh stabilised by the incorporation of a spiral of low pressure polyethylene. Prostheses of the latter material were implanted intraperitoneally for 3–4 weeks. In spite of numerous experiment, early and late complications were found in 50 % and infection and scar shrinkage with stenosis in some 25 %.
