Critical Evaluation of the Japanese “Early Gastric Cancer” Classification

Summary

The analysis of Japanese statistics shows that the classification for early gastric cancer as suggested by the Japanese Society for Gastroenterological Endoscopy is not practicable due to immense subjective variations. Since this classification is not reproducable and thereby not reliable a new classification is suggested differentiating only between polypoid and ulcerous forms. This is sufficient for practical endoscopic purposes and for the evaluation of a prognostic index.

Zusammenfassung

Die Analyse japanischer Statistiken zeigt, daß die 1962 von der Japanischen Gesellschaft für gastroenterologische Endoskopie vorgeschlagene Typeneinteilung der Magenfrühkrebse bei der praktischen Anwendung zu großen subjektiven Schwankungen unterliegt. Sie ist nicht hinreichend reproduzierbar, hat eine zu geringe Reliabilität. Daher wird vorgeschlagen, nur noch zwischen polypoiden und ulzerösen Formen zu unterteilen. Dies genügt für praktische endoskopische Zwecke und zur Klärung der Beziehungen zwischen makroskopischem Frühkrebstyp und Prognose.
